CoQ10 vs Fish Oil for Aging Dog Heart Support: What the Evidence Actually Says

For an aging dog's heart, fish oil has real trial support; CoQ10's evidence is still catching up.
Does an aging heart actually need CoQ10, or is fish oil doing the real work?
I get asked this in exam rooms more than almost any other supplement question, usually right after a diagnosis of myxomatous mitral valve disease or early dilated cardiomyopathy. The owner has already read three forum threads and wants to know which capsule to buy first. My honest answer is that these two supplements aren’t interchangeable, and treating them as competitors misses the point — they’re supporting different parts of a very different problem.
A twelve-year-old Cavalier King Charles Spaniel named Winnie came in for her annual senior workup at 8.2 kilograms, down about half a kilogram from the year before. Her owner hadn’t noticed much beyond a soft cough after she settled down for the night and a reluctance to finish her usual after-dinner lap around the block. Winnie already had a grade III/VI murmur and mild left atrial enlargement on echo — textbook early MMVD for the breed. Her owner had read that CoQ10 could “strengthen the heart muscle” and wanted to start it that week, alongside or instead of the omega-3 capsule her regular vet had already recommended. What the CoQ10 trials actually show
CoQ10 is a naturally occurring cofactor in mitochondrial energy production, concentrated in tissues with high energy demand — including cardiac muscle. The logic for supplementing it in heart disease is straightforward: MMVD hearts are working harder, so more CoQ10 should help them work more efficiently. The trial evidence in dogs is thinner than that logic suggests.
A randomized, double-blinded, controlled trial in the American Journal of Veterinary Research found that a daily dose of 200 mg of water-soluble CoQ10 reliably raised plasma CoQ10 concentration in dogs with MMVD — the supplement clearly gets absorbed. What it didn’t establish is whether that higher plasma level changes disease progression. A separate placebo-controlled study of 43 dogs across ACVIM stages B2 through C/D, tracked on PMC, used the same 100 mg twice-daily dose for three months and found no significant change in echocardiographic measurements, NT-proBNP, or troponin I — the markers cardiologists actually use to grade severity and response. Where CoQ10 did move the needle was on the white blood cell count: supplemented dogs in congestive heart failure showed a shift toward fewer neutrophils and more lymphocytes relative to placebo, a pattern consistent with less systemic inflammation. Owners in that study also reported subjective improvement, though the authors didn’t statistically analyze that part.
Cardiologists at Tufts Cummings School of Veterinary Medicine are similarly cautious, noting there are no definitive canine or feline studies establishing a clinical benefit, though some human dilated cardiomyopathy research is suggestive. That’s a fair summary of where I land, too: CoQ10 looks safe, it clearly raises blood levels, and it may modulate inflammation at the margins. It has not been shown to change how MMVD or DCM actually progresses in a dog.
What fish oil has actually shown in dogs with congestive heart failure
Fish oil’s case is built on a narrower but more concrete finding: cardiac cachexia. Dogs in congestive heart failure lose lean muscle mass through a cytokine-driven process, and that loss is associated with meaningfully shorter survival — this isn’t a cosmetic concern, it’s a prognostic one. Freeman’s landmark trial, published in the Journal of Veterinary Internal Medicine, found that EPA and DHA supplementation reduced interleukin-1 concentrations and improved markers of cachexia compared to placebo in dogs with heart failure. That’s a mechanism I can actually explain to an owner: omega-3s appear to blunt the inflammatory cytokines driving muscle wasting, and reduced cytokine activity has been linked to better survival odds in CHF dogs. Fish oil also carries independent support for reducing arrhythmia risk and easing the workload of a remodeling heart, which is why it shows up in most cardiac diets already. One caveat worth saying plainly: fish oil doesn’t prevent heart disease in dogs the way it’s sometimes marketed to do in people, because dogs don’t typically develop the coronary artery disease that omega-3s are protecting against in that context. Its job here is supportive — managing inflammation and muscle preservation around a valve or muscle problem that already exists, not reversing the underlying lesion.

The strongest counterargument is that CoQ10 isn’t unsafe, it’s cheap relative to some fish oil products, and the inflammatory marker shift in the PMC data is a real signal, even if it’s not a survival endpoint. I take that seriously — CoQ10 isn’t a supplement I’d talk an owner out of if their dog is already stable and they want to add it on top of fish oil and their prescribed cardiac medications. Confirm the diagnosis and stage first — a cardiologist’s echo report tells you far more than a supplement label does.
